I have uploaded a prototype of a simple character sheet for this game, and I would appreciate feedback on it from anyone who has played or is playing the game. Are there ways the character sheet could be improved to be more useful to players? Are there things to add or ways to reformat it? 

Once I have finalized the design after feedback I will upload a form-fillable PDF of the final sheet. It will be available only to those who have paid for the game.
